This. If my jungler is mana dependent (mumu for example), I'll let him have blue and get an early wand+tear and THEN go hat. It makes me a little less useful in terms of damage output early game, but with Karthus I never have problems farming my lane except against Zyra or a good Swain, so I don't worry much about it. If the game looks lopsided really early, like it might be a 20-25 minute game, I'll skip the tear and just rush hat, but it's rare that a game is so lopsided by my first back (generally around level 6, maybe a bit later if I haven't managed to get a kill/assist/really good farm by then--I generally want to back and buy at least one item+2 wards mid-lane on my first back, so I'll delay it until I can if I can't right at 6). And in the games where I don't build a tear-->AA, I wind up with slightly less AP with a full build than I get with it, so if I'm in doubt I go with AA.
