forgetting to swap my d-scan settings back when im done with a 5 degree scan or w/e
Switching from a long range to a short range ship, or short to long, and forgetting to change my orbit / keep at range distances. I've had several truly retarded losses because of that.
Forgetting to pull drones to degaress
Accidentally unaligning due to doubleclick
Running out of cap just before target death, allowing them to warp out.
Falling asleep at a 0.0 gate.
Not a rookie mistake exactly, but I've found myself making it recently. With the introduction of shift-click to overheat on SiSi and spending time testing active-tanked BC fits plus AF changes, I've now got a shift-click overheat reflex. Which shuts off modules on TQ. FML. Accidentally managed to turn off absolutely everything in the middle of a heated fight a few days ago. Still won, but it was close.
In actual rookie mistakes, I still find myself making gate-to-gate warps in 0.0 without d-scanning first sometimes.

Warped my ship to a bookmark inside the docking ring, got station aggro when I aggressed a ship there, then got permabumped by the station model when warping out and lost the ship to gate guns. /o\

Gate guns tend not to forgive derps very easily. Was going after haulers moving into a WH one day, had my bomber sitting cloaked just off the WH on the lowsec side. Hauler came out, went GCC on him but he was stabbed so he got away. My next though was that I needed something with scram instead of point so I jumped to the next system to grab a new ship, got my bomber blapped by gate guns. AND I forgot to loot my own wreck until literally 2 hours 5 mins later by which time it was too late.
I fly minmatar mainly and I fly with phased plasma loaded by default.
I will catch something on scan then warp to it and when I land i find I have completely forgotten to change ammo based on what I was warping too. So lazy.
I killed a Nighthawk with Phased Plasma once. Took ages~

Let me expand on that: I sometimes make gate-to-gate warps in -occupied- 0.0 systems without d-scanning first.
Ironically, this netted me a Sabre kill the other day, landed in his bubble. FNC vs Sabre, expected to die in a fire so ENGAGE! He melted before I even hit structure. Turns out he had all nanos low, no web, and only a MWD.

Warping to a cyno looking for quick easy kill and finding self at pos with a titan.
